I agree that the Sega are great, especially the big metal console sticks. I have always had to modify mine to get more than one button to work.
The pad for the CD32 worked for me without modifications, as I remember the few times I used it.
In truth, I don't do a lot of joystick-type gaming, but it seems that derived buttons (those other than those hard-wired to the port) might be interpretted differently from one game to the next. So far, I haven't taken the time to confirm this.
I believe there are several (different?) methods to modify Sega pads. One set of instructions come with the docs for ADoom.
